Required Pre-Conditions for Service Application

• Completion of the Service Request Form (application)
• Submission of a valid identification document: Passport, National ID, or Tax Identification Certificate
• If applying through an agent, submission of a power of attorney and identification document of the applicant
• Submission of a notarized or certified original document and its copy
• Two passport-size photos (3x4 cm) (if applicable for all authorized representatives)
• Payment of the service fee
• If applicable, submission of property ownership details:
  • Sale or donation agreement
  • Legal certificate issued by an authorized body confirming property rights
  • Lease agreement or related legal documents
• If leasing, submission of lease contract
• Proof of payment of due fees related to the lease agreement

Additional Documents for Specific Cases:

• Submission of a new lease contract if the previous lease has expired or been terminated
• If the property is under construction, the construction status should be no less than 50% completed
• Submission of bank clearance for payments made (if applicable)
• Official letter from the bank indicating the amount paid
• Completion of outstanding debts as per the contract
• Submission of updated lease contract details
• Official letter from the agent confirming the application submission
• Submission of the required documentation for shared housing/condominium ownership
• Legal proof of shared ownership (if applicable)

Additional Notes:

• If the residential building has been under construction for over 5 years, an explanation of delays and supporting documentation should be submitted
• Proof of inheritance or rights to the property should be submitted if applicable
• Divorce settlement and related legal documents confirming the division of assets
